Register mii_bus with read and write callbacks to allow the 'mii'
command to work. Use a timeout of 10 ms to wait for the R/W
operations to complete.

Replace 'phys_addr_t iobase' with 'struct ftmac100 *ftmac100' in
struct ftmac100_data. It allows to remove casting in a number of places.

Since priv-&gt;iobase is phys_addr_t, use phys_to_virt() to make
a pointer from it.

priv-&gt;iobase was declared as phys_addr_t which is now a 64-bit
address. In a 32-bit build, this causes the following warning
seen when building ftmac100.c:

  warning: cast to pointer from integer of different size [-Wint-to-pointer-cast]

Cast priv-&gt;iobase with uintptr_t.

Move this out of the common header and include it only where needed.  In
a number of cases this requires adding "struct udevice;" to avoid adding
another large header or in other cases replacing / adding missing header
files that had been pulled in, very indirectly.   Finally, we have a few
cases where we did not need to include &lt;asm/global_data.h&gt; at all, so
remove that include.

We use 'priv' for private data but often use 'platdata' for platform data.
We can't really use 'pdata' since that is ambiguous (it could mean private
or platform data).

Rename some of the latter variables to end with 'plat' for consistency.
