It is confusing to have both "$(PHASE_)" and "$(XPL_)" be used in our
Makefiles as part of the macros to determine when to do something in our
Makefiles based on what phase of the build we are in. For consistency,
bring this down to a single macro and use "$(PHASE_)" only.

Pull the functionality of UDC uclass that operates on plain udevice
and does not use this dev_array array into separate functions and
expose those functions, so that as much code as possible can be
switched over to these functions and the dev_array can be dropped.

Fixes commit 013116243950 ("dm: usb: create a new UCLASS ID for USB gadget
devices")

The UCLASS_DRIVER for id UCLASS_USB_GADGET_GENERIC needs to be declared
even for platforms that do not enable DM_USB_GADGET. Otherwise the driver
for their usb peripheral controller fails to bind.

UCLASS_USB_DEV_GENERIC was meant for USB devices connected to host
controllers, not gadget devices.
Adding a new UCLASS for gadget devices alone.

Also move the generic DM code for USB gadgets in a separate file for
clarity.

<title>SPDX: Convert all of our single license tags to Linux Kernel style</title>

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from.  So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry.  Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.

In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.

This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents.  There's also a few places where I found we did not have a tag
and have introduced one.

After consulting with some of the SPDX team, the conclusion is that
Makefiles are worth adding SPDX-License-Identifier tags too, and most of
ours have one.  This adds tags to ones that lack them and converts a few
that had full (or in one case, very partial) license blobs into the
equivalent tag.

Added udc-core.c from linux kernel 3.19-rc1 (97bf6af1f9) to u-boot.
This will be adapted to work with u-boot in the
following patches.
Adding support for udc will help to seamlessly port dwc3 driver from
linux kernel to u-boot (since dwc3 uses udc-core) and it'll also help
to add support for multiple gadget controllers to be functional at the
same time.
All other gadget drivers can also be adapted to use udc-core.
