In CP115, comphy4 can be configured into SFI port1
(in addition to SFI0). This patch adds the option
described above.

In addition, rename all existing SFI/XFI references:
COMPHY_TYPE_SFI --&gt; COMPHY_TYPE_SFI0

No functional change for exsiting configuration.

Currently, all comphy definitions are PHY_TYPE_XX and PHY_SPEEED_XX.

  USB host and device controller, but on PHY level the
  configuration is the same.
- The new type supports both USB device and USB host
- This patch is cherry-picked from u-boot-2015 as-is.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from.  So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry.  Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.

In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.

This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents.  There's also a few places where I found we did not have a tag
and have introduced one.

This type tells u-boot to preserve the COMPHY settings as is
it is usefull in situations where the COMPHY was initialized by
earlier firmware.
Note that IGNORE is different from UNCONNECTED since setting
UNCONNECTED type will disconnect the COMPHY in the COMPHY MUX
which is a desired behaviour

UTMI_PHY_TO_USB_HOST was used in USB3 UTMI dts node only, but there will
be USB2 UTMI dts node for some SoCs that have got USB2 controller, so rename
TO_USB_HOST to TO_USB3_HOST to distinguish TO_USB2_HOST in later on patches.
