fs: fat: structure for name and extension

The short name and extension of FAT files are stored in adjacent fields of
the directory entry. For some operations like calculating a checksum or
copying both fields it is preferable to treat both as one structure.

Change the definition of the directory entry structure to include a
structure comprising the name and the extension field.

This resolves Coverity CID 316357, CID 316350, CID 316348.

The FAT file system does not have a UUID but a 4 byte volume ID.
Let the fsuuid command show it in XXXX-XXXX format.

The FAT filesystem implementation uses several marcros referring to a magic
variable name mydata which renders the code less readable. Eliminate one of
them which is only used for a debug() statement.

Use log_debug() instead of debug().

Rename function next_cluster() to fat_next_cluster() and export it.

When creating a new directory entries we should reuse deleted entries.
This requires re-scanning the directory.

fs: fat: support unlink

In this patch, unlink support is added to FAT file system.
A directory can be deleted only if it is empty.

In this implementation, only a directory entry for a short file name
will be removed. So entries for a long file name can and should be
reclaimed with fsck.

fs: fat: fix wrong casting to unsigned value of sect_to_cluster()

After the commit 265edc03d5a1 ("fs/fat: Clean up open-coded sector
<-> cluster conversions"), it is hung up writing new file to FAT16
disk with more than 19 files in armv7. It is because result value
of sect_to_cluster() is not proper by casting from signed value to
unsigned value. Fix the wrong casting of sect_to_cluster().
