Remove duplicate definitions in include/lxt971a.h.

Remove duplicate registers and bits definitions in
include/lxt971a.h for standard MII registers, and
use values in include/miiphy.h instead.

This commit gets rid of a huge amount of silly white-space issues.
Especially, all sequences of SPACEs followed by TAB characters get
removed (unless they appear in print statements).

Also remove all embedded "vim:" and "vi:" statements which hide
indentation problems.

- split out the LXT971A PHY from ns_9750_eth.h
- split the dm9161 phy part out of at91rm9200_ether.c
