lib: Remove inclusion from these files

After some header file cleanups to add missing include files, remove
common.h from all files in the lib directory. This primarily means just
dropping the line but in a few cases we need to add in other header
files now.

lib: parameter check in hash_calculate

If hash_calculate is invoked with region_count = 0, it will try to hash
INT_MAX regions. We should check this parameter.

* Avoid a comparison with different signedness.
* Check that region_count is at least 1.
* Avoid a superfluous assignment.

This file has a lot of conditional code and much of it is unnecessary.
Clean this up to reduce the number of build combinations.

lib: Rename rsa-checksum.c to hash-checksum.c

rsa-checksum.c sontains the hash_calculate() implementations. Despite
the "rsa-" file prefix, this function is useful for other algorithms.

To prevent confusion, move this file to lib/, and rename it to
hash-checksum.c, to give it a more "generic" feel.
