At present devices use a simple integer offset to record the device tree
node associated with the device. In preparation for supporting a live
device tree, which uses a node pointer instead, refactor existing code to
access this field through an inline function.

The nios2-io.h defines hardware registers and bits of several FPGA
IP cores. It could be divided in to the specific drivers, including
altera timer, altera sysid, altera uart and altera jtag uart. The
altera pio and altera spi drivers use their own hardware definitions.
The removal of nios2-io.h will help modularity and maintenance.

Replace the in-place ad-hoc implementation of serial_puts() within
the drivers with default_serial_puts() call. This cuts down on the
code duplication quite a bit.

Remove the support for not-CONFIG_SERIAL_MULTI part from serial
port drivers and some board files. Since CONFIG_SERIAL_MULTI is
now enabled by default, that part is a dead code. Remove it.

Implement support for CONFIG_SERIAL_MULTI into altera serial driver.
This driver was so far only usable directly, but this patch also adds
support for the multi method. This allows using more than one serial
driver alongside the altera driver. Also, add a weak implementation
of default_serial_console() returning this driver.

The outx/writex macros were using writex(addr, val) rather than
the standard writex(val, addr), resulting in incompatibilty with
architecture independent components. This change set uses standard
parameter order.
