Add option to set shell prompt string from menuconfig and migrate
boards globally.
The migration is done as follows:
- Boards that explicitly and unconditionally set CONFIG_SYS_PROMPT had the
entry moved to their defconfig files.
- Boards that defined some kind of #ifdef logic which selects the
CONFIG_SYS_PROMPT (for example qemu-mips) got an #undef CONFIG_SYS_PROMPT
right before the #ifdef logic and were left alone.
- This change forces CONFIG_SYS_PROMPT to be a per board decision, and thus
CONFIG_SYS_PROMPT was removed from all <soc>_common.h and <arch>_common.h
files. This results in a streamlined default value across platforms, and
includes the following files: spear-common, sunxi-common, mv-common,
ti_armv7_common, tegra-common, at91-sama5_common, and zynq-common.
- Boards that relied on <arch/soc>_common.h values of CONFIG_SYS_PROMPT were
not updated in their respective defconfig files under the assumption that
since they did not explicitly define a value, they're fine with whatever
the default is.
- On the other hand, boards that relied on a value defined in some
<boards>_common.h file such as woodburn_common, rpi-common,
bur_am335x_common, ls2085a_common, siemens_am33x_common, and
omap3_evm_common, had their values moved to the respective defconfig files.
- The define V_PROMPT was removed, since it is not used anywhere except for
assigning a value for CONFIG_SYS_PROMPT.

This sets the default commands Kconfig to match
include/config_cmd_default.h commands in the common/Kconfig and removes
them from include/configs.

The last users of CONFIG_KGDB_SER_INDEX were removed more than 3 years
ago in commits 550650ddd0 and bf16500f79, either kgdb subsystem should
care about this parameter or it should be gone completely.

The definitions for CONFIG_SYS_PROMPT are varied with little reason other
than to display the board name. Over half the definitions are "==> ", so
make this the default. The rest of the boards remain unchanged to avoid
breaking any external scripts expecting a certain prompt.

Since the lcd code was compiled unconditionally for pxa also add
CONFIG_PXA_LCD to the boards using this framebuffer. Since
driver/video contains video and lcd drivers, add lcd to the name
to make clear it belongs to common/lcd.c.

Some of the boards still used the old PXA_MMC driver instead of the
new generic one. Use the new one instead so the old can be removed
and the generic MMC framework can be properly used.

Make use of the newly implemented weak default_serial_console in
the serial_pxa driver. This removes all reimplementations of this
function from board files.

CONFIG_STACKSIZE is not referenced anywhere except on AVR32, but present
in most ARM board config files.
IRQs are only enabled for 1 config, so remove the unused config options
for IRQ and FIQ stack size as well.

Exactly one board has defined CONFIG_SYS_PROMPT_HUSH_PS2 to a value
different than "> " which is vision2. I have Cc'd the maintainer here
as I strongly suspect this is a bug rather than intentional behavior.

We provide a default table of { 9600, 19200, 38400, 57600, 115200 }
in <config_fallbacks.h> which mkconfig places after <configs/...h> in
the generated config file. This is used when a board has not set its
own table.

This renames BOARD_LATE_INIT to CONFIG_BOARD_LATE_INIT.
Along the way it removes some leftover
#define BOARD_LATE_INIT 1
and adds some basic documentation for board specific
callbacks in README.

names. Following are the changes:

For ARM systems, before ELF relocation was introduced,
CONFIG_SKIP_RELOCATE_UBOOT coul be used to prevent *COPYING* the
U-Boot image from whereever it was loaded to it's link address
(CONFIG_SYS_TEXT_BASE). The name was badly chosen, as no relocation
was performed at all, it was just a memcpy().
With ELF relocation, this does not work like that any more, and
related boards need to be fixed anyway. So don't keep this relict any
longer.

CONFIG_SYS_GBL_DATA_SIZE has always been just a bad workarond for not
being able to use "sizeof(struct global_data)" in assembler files.
Recent experience has shown that manual synchronization is not
reliable enough. This patch renames CONFIG_SYS_GBL_DATA_SIZE into
GENERATED_GBL_DATA_SIZE which gets automatically generated by the
asm-offsets tool. In the result, all definitions of this value can be
deleted from the board config files. We have to make sure that all
files that reference such data include the new <asm-offsets.h> file.
No other changes have been done yet, but it is obvious that similar
changes / simplifications can be done for other, related macro
definitions as well.

Fixing the get_timer function to return time in miliseconds instead of
ticks. Also fixed PXA boards to use the conventional value of 1000 for

The pxa255_idp being an old unmaintained board showed several issues:
1. CONFIG_INIT_CRITICAL was still defined.
2. Neither CONFIG_MAC_PARTITION nor CONFIG_DOS_PARTITION was defined.
3. Symbol flash_addr was undeclared.
4. The boards lowlevel_init function was still called memsetup.
5. The TEXT_BASE was still 0xa3000000 rather than 0xa3080000.
6. Using -march=armv5 instead of -march=armv5te resulted in lots of
'target CPU does not support interworking' warnings on recent compilers.
7. The PXA's serial driver redefined FFUART, BTUART and STUART used as
indexes rather than the register definitions from the pxa-regs header
file. Renamed them to FFUART_INDEX, BTUART_INDEX and STUART_INDEX to
avoid any ambiguities.
8. There were several redefinition warnings concerning ICMR, OSMR3,
OSCR, OWER, OIER, RCSR and CCCR in the PXA's assembly start file.
9. The board configuration file was rather outdated.
10. The part header file defined the vendor, product and revision arrays
as unsigned chars instead of just chars in the block_dev_desc_t
structure.

Explicitly add in default CONFIG_BOOTP_* options where cmd_confdefs.h
used to be included but CONFIG_BOOTP_MASK was not defined.
