This converts the following to Kconfig:
CONFIG_SYS_MONITOR_LEN
To do this, we set a default of 0 for everyone because there are a
number of cases where we define CONFIG_SYS_MONITOR_LEN but the only
impact is that we set TOTAL_MALLOC_LEN to be CONFIG_SYS_MALLOC_LEN +
CONFIG_ENV_SIZE, so we must continue to allow all boards to set this
value. Update the SPL code to use 200 KB as the default raw U-Boot size
directly, if we don't have a real CONFIG_SYS_MONITOR_LEN value.

- Make all users of CUSTOM_SYS_INIT_SP_ADDR reference SYS_INIT_SP_ADDR
- Introduce HAS_CUSTOM_SYS_INIT_SP_ADDR to allow for setting the stack
pointer directly, otherwise we use the common calculation.
- On some platforms that were using the standard calculation but did not
set CONFIG_SYS_INIT_RAM_SIZE / CONFIG_SYS_INIT_RAM_ADDR, set them.
- On a small number of platforms that were not subtracting
GENERATED_GBL_DATA_SIZE do so now via the standard calculation.
- CONFIG_SYS_INIT_SP_OFFSET is now widely unused, so remove it from most
board config header files.

In some cases, we define CONFIG_SYS_INIT_SP_ADDR differently for SPL or
full U-Boot. This case should be making use of CONFIG_SPL_STACK, as
that's what that variable is for. In a few other cases we define
CONFIG_SPL_STACK directly to CONFIG_SYS_INIT_SP_ADDR, but do not need to
as the code handles this correctly, normally.

With the commit that moves the BOOTCOMMAND to Kconfig:
970bf8603b ("Convert CONFIG_USE_BOOTCOMMAND et al to Kconfig")
some stray ifdefs have been left in the header files which
are now useless.
Clean up the include files to remove these lines.

This adds a defconfig for sama5d2_xplained board to get environment from
eMMC. The defconfig is made from sama5d2_xplained_mmc_defconfig, with
'bootcmd' and 'bootargs' changed to kernel, device-tree and rootfs from
eMMC. The environment is expected to be found in eMMC's FAT
partition.

We have the following cases:
- CONFIG_NR_DRAM_BANKS was defined, migrate normally
- CONFIG_NR_DRAM_BANKS_MAX was defined and then used for
CONFIG_NR_DRAM_BANKS after a check, just migrate it over now.
- CONFIG_NR_DRAM_BANKS was very oddly defined on p2771-0000-* (to 1024 +
2), set this to 8.

For sama5d2_xplained_spiflash_defconfig, we have the demo layout
as presented on this link:
http://www.at91.com/linux4sam/bin/view/Linux4SAM/Sama5d2XplainedMainPage#SPI_eMMC_Flash_demo_Memory_map
on SPI Flash (4 Mbyte) we have Bootstrap (second level bootloader), U-boot + env
and kernel+dtb we keep on eMMC on single partition in /boot directory, formatted
with ext4.
Thus, changing the boot command to reflect this demo for the spiflash config,
and fixing up bootargs. Sama5d2_xplained does not have NAND flash, so the
bootargs were completely wrong.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

Migrate the option CONFIG_SPL_FRAMEWORK and make this gate most of the
current set of options we have in Kconfig. We will need to have some
options available for SPL and !SPL_FRAMEWORK so this is important. In a
few cases we re-order existing options so that we have less escapes from
the SPL_FRAMEWORK guard.

To remove the assignment of CONFIG_SYS_EXTRA_OPTIONS option,
which is deprecated, use the CONFIG_XXXX_BOOT options to
indicate the boot media, and the SoC is selected by the board.

Because before switching to a lower clock source, we must switch
the clock source first instead of last. So before configuring the
PMC_MCKR register, invoke at91_mck_init_down() first.
As said in datasheet, the the size of SPL must not exceed the maximum
size allowed(64Kbytes).

We repeated partial moves for CONFIG_SYS_NO_FLASH, but this is
not completed. Finish this work by the tool.
During this move, let's rename it to CONFIG_MTD_NOR_FLASH.
Actually, we have more instances of "#ifndef CONFIG_SYS_NO_FLASH"
than those of "#ifdef CONFIG_SYS_NO_FLASH". Flipping the logic will
make the code more readable. Besides, negative meaning symbols do
not fit in obj-$(CONFIG_...) style Makefiles.
This commit was created as follows:
[1] Edit "default n" to "default y" in the config entry in
common/Kconfig.
[2] Run "tools/moveconfig.py -y -r HEAD SYS_NO_FLASH"
[3] Rename the instances in defconfigs by the following:

Now, CONFIG_GENERIC_MMC seems equivalent to CONFIG_MMC.
Let's create an entry for "config GENERIC_MMC" with "default MMC",
then convert all macro defines in headers to Kconfig. Almost all
of the defines will go away.
I see only two exceptions:
configs/blanche_defconfig
configs/sandbox_noblk_defconfig
They define CONFIG_GENERIC_MMC, but not CONFIG_MMC. Something
might be wrong with these two boards, so should be checked later.
Anyway, this is the output of the moveconfig tool.

Move CONFIG_SDHCI to Kconfig and rename it to CONFIG_MMC_SDHCI.
My motivation for the rename is, ultimately, to make all the MMC
options prefixed with MMC_ and SDHCI options with MMC_SDHCI_,
like Linux.
